What To Bring To Your Pet's First Appointment:
PREVIOUS RECORDS
We request that you bring any records you have for your pet. Vaccination records are especially important to have ready to go for the appointment. If you need help retrieving the pet's records from another hospital or animal shelter, please feel free to give us a call at (614)876-0455 and we would be happy to assist you!
NEW CLIENT FORM
Attached below is our new patient/new client form. We ask that if this is the first time you are bringing your pet to us, you fill out this form prior to your appointment. The attached document is interactive, so you may fill it out online and email it to us OR print it out and bring it to your pet's appointment.
FECAL
SAMPLE
We request that you bring a fecal sample to your pet's first appointment with us to be tested for parasites. Typically this is brought in a zip-lock bag or a container that you will not want back. The fecal sample must be collected within 24-hours of the appointment and refrigerated to ensure the most accurate lab results.
